ASA Official Entry Form

ASA Individual registration and photo ID

Official ASA Championship Roster at check in with all signatures (players, parents if player under 18, Commissioner or designee)

All coaches in dugout must have proof of ASA background check and one coach must have proof of ACE certification

Players will be required to sign in prior to participating in any games
